Objectives

At the end of the course, the student will be able to:1. Explain the different models and principles behind community asset-building and empowerment.2. Improve competence in addressing social determinants of health for older persons, through social prescribing and community action.3. Appraise ways to activate community-wide volunteerism.4. Increase primary care partnerships in community care and health promotion efforts for older persons.5. Draft a plan applying these models/principles to any existing health promotion and community care program for older persons.
