Objectives
Upon completion of the course, students should be able to:
1. Describe the profile of older patients and their caregivers in various care environment/services available in Singapore.
2. Discuss the psychosocial, and self-management behaviours among older patients, as well as the associated factors/interventions that can influence medication management, adherence and use outcomes by older adults in the community.
3. Discuss the potential challenges and issues regarding chronic medication management by older patients in the community in a holistic manner, with regards to ethical considerations, medical, psychosocial and economic factors).
4. Identify situations and appropriate communication skills (and counselling framework) that can be applied during pharmacist-patient interactions in community-based pharmaceutical care services to promote collaborative self-management of illness, treatment and health by older adults and/or their caregivers.
5. Discuss signs and symptoms that require referrals, with due considerations of appropriate resources to refer to and the immediate response to be provided.
